West Berkshire Mencap have created ‘free downloadable’ resources for individuals to record ‘My Wishes’ on their own, or with support of a family member or a carer; about what they want to happen to their possessions and their funeral when sadly they are no longer here, something everybody must think about for when we are no longer here.
The attached documents ‘My Wishes’ and ‘My Funeral’ were created by West Berkshire Mencap with help from individuals with learning disabilities who attend WBMencap Adult services; the projected was funded by West Berkshire Learning Disability Partnership Board.
Please note: My wishes is not a legally binding document; it is just ‘My Wishes’ and to be used by Adults with Learning Disabilities who have Mental Capacity to make decisions about their possessions and their funeral wishes.
